Chronic Care Management
Do you struggle with high blood pressure, diabetes, or similar issues. Ask about the Comet MD RPM program. 100% covered by most insurances, our RPM program will provide you with near real-time monitoring of your issues by trained professionals. You’ll get easier more consistent access to a care team that will help you feel better and stay in control!
Costs & Insurance
We do take most insurances; however, you should check with your provider first if you are unsure. We strongly feel that cost and insurance shouldn’t be a hinderance to finding care. To that end we offer Low Cost rates from $19 to $99** depending on the complexity of your case.
*Medications we are unable to prescribe.
- DEA-controlled medications (including ADHD stimulants & anti-anxiety benzodiazepines) AND
- Medications for erectile dysfunction, hair loss or weight loss.) Medications intended for detox (e.g., Methadone.) Medications that are often abused such as pseudoephedrine and most muscle relaxants. Sleep medications, medical marijuana, etc…

We do not provide care for:
Emergency situations (broken bones, significant bleeding, heart attack, suicidal thinking) CALL 911 in case of Emergency!
